TC Family Clinic is a family practice in Singapore dedicated to men, women, children and elders. We have been a trusted practice for over 20 years, serving and caring for families since 1999. We believe in personalized care, and we work with patients to understand and address every individual’s unique needs.
With our team of trained and experienced health professionals, we are focused on behaviour modification and achieving the best outcomes for our patients. We believe that strong therapeutic relationships are crucial to help our community live well and plan ahead for a high quality of life.
Dr. Tammy Chan

Dr. Tammy Chan is the primary family physician at TC Family Clinic. Dr. Chan received her Bachelor of Medicine, Bachelor of Surgery (MBBS) from the National University of Singapore Yong Loo Lin School of Medicine in 1991, and completed her Graduate Diploma in Occupational Medicine (GDOM) in 2000. She later completed her Graduate Diploma in Mental Health (GDMH) from the National University of Singapore in 2014.
Dr. Chan is actively involved in various initiatives and programs relating to chronic disease care, diabetes, and mental health.
In 2018, she was elected to the executive committee for the SingHealth Delivery on Target Primary Care Network for private GPs – a regional health system collaboration for integrating community chronic disease management.
Dr. Chan is also involved in the Institute of Mental Health GP Partnership Program, as well as the Community Mental Health Program in collaboration with JurongHealth. She has also received her certification of Obstetric & Gynaecology Ultrasonography in 2008.
Dr. Chan is also an active participant in medical professional engagement groups.
She has had experience serving on the executive committee of the Singapore Medical Association (SMA), serving as the chairman of the SMA Private Practice Committee from 2016 to 2018, as well as Honorary Secretary and SMA spokesperson from 2012 to 2015. She currently holds the position of second Vice President of the SMA.
Dr. Chan has also been a member of the Ministry of Health National GP Advisory Panel from 2014 to 2017.

More recently, Dr. Chan represents primary care and specialists from the private medical sector with her recent appointment under MOH to serve in the Chief Medical Informatics Office (CMIO) of MOH Singapore, involved in the oversight of health IT policy development.
She is also appointed to represent the Singapore Medical Association in the Healthier SG Implementation Main Work Group.
She was previously appointed in the Disease Management Workgroup, Diabetes Prevention and Care Taskforce in Singapore.
Dr. Chan’s specialization interests are in mental health, chronic disease management, diabetes, elderly care and gynaecology.
